  • Artificial Pancreas Device Systems
  • Coronary CT Angiography (CCTA)
  • Cryosurgical Ablation of Miscellaneous Solid Tumors Other Than Liver, Prostate, or Dermatologic Tumors
  • Genetic and Protein Biomarkers for the Diagnosis and Cancer Risk Assessment of Prostate Cancer
  • Genetic Testing for Hereditary Breast/Ovarian Cancer Syndrome (BRCA1 or BRCA2)
  • KRAS, NRAS, and BRAF Variant Analysis in Metastatic Colorectal Cancer

All category III CPT Codes, including new 2018 codes, are non-covered unless they are explicitly described as “medically necessary” in a BCBSMA medical policy.
